The Australian Transport Safety Bureau (ATSB) found evidence of ineffective coordination and monitoring by the crew and harbour pilots on board the 8,814-teu Maersk Shekou (built 2010) during the accident at Fremantle last year. The 236-gt tall ship Leeuwin II (built 1986) was badly damaged and two crew injured in the incident.
